Admissions

To place your child on the waitlist please complete the waitlist package. If there is space for enrollment, then you may complete the admissions package. Once completed, these forms can either be scanned and emailed back to the office hazelcreekoffice@gmail.com or delivered to the school in person. For both the waitlist and enrollment, we request a one-time $175 processing fee. The processing fee is per family.

The Hazel Creek Montessori Admissions Package includes our Admissions Form, an Authorization Form for Direct Payments, and the State of Washington Certificate of Immunization Status.
